HOW DO I....
REGISTER WITH THE PRACTICE?

We are pleased to welcome new patients who have moved into our practice area.

If possible, please bring your medical card to the surgery when you register. If your card is not available, you will be asked to complete a simple form instead.

All new patients are encouraged to attend for a health check with the practice doctor and a healthcare assistant. New patients must be seen by a doctor before repeat prescriptions can be issued.

TEMPORARY RESIDENTS

You are welcome to attend the surgery by appointment if you are staying in the practice area for a short period.

CHANGE MY PERSONAL DETAILS?

If you change your name, address or telephone number, please inform the receptionist as soon as possible and complete the appropriate form.
